North East England

CHOICES  CARE has been supporting people in the North east of England since 2003 primarily in Stockton and Durham. In the summer of 2009 it is expected that CHOICES  will also expand into Sunderland, taking on contracts from the Trust and Local Authority to manage 7 existing Registered Care Homes

Most of the people who use our services have Learning Disabilities.  However, we also support clients in the area with Mental Ill Health and Autism Spectrum Disorder, and physical disabilities

The range of services we provide changes according to the individual needs of the people who use our services.

Our high standard of care and support packages extends from between 24hours per day,  7 days a week to just a few hours of care when required. Delivered in a wide variety of settings ranging from Registered Care Homes , Supported and independent Living settings within houses of multiple occupancy and single tenancies, also to adults living in general needs housing and floating support, outreach care at home and family respite services in family homes, day centers and colleges  in non- building based models

Funding is via, Local authority, supporting People, direct payments and ILf and continuing health care and all referrals recurrently received from the Social Work Team

Both the Registered care Home and the domiciliary care services in Stockton and Durham have been rated as excellent by CQC and these reports and ratings are available on the CQC professional website.
