Adults with Care and Support Needs

The levels of care and support we provide depends on an assessment of the person's needs, carried out by the person, their care manager (social worker, community nurse), the person's family, relatives and friends and CHOICES CARE.

The Personal Plan and the level of support vary depending on the person's needs, views, opinions and wishes.

Our main aim is to enhance the quality of life of the people who use our services, whilst ensuring that the support we provide is flexible enough to allow for changing needs or circumstances. Person Centred Care and support is at the heart of all we do.

The people we support are:

  • People with Physical or Learning Disabilities including those with a history of homelessness
  • People with Long Term Medical Conditions
  • People with Mental Ill Health
  • People recently Discharged from Hospital
  • Older People

Some of the settings that we provide support in are as follows:

  • At home
  • In their local community
  • At day care centres
  • In college

What we aim to achieve by working closely with the people we support and their close environment is:

  • We support people to live as independently as possible
  • We want the people we support to feel they have had as much influence as possible in deciding their Personal Plan
  • We want the people we support to feel the support we are providing is contributing to how they choose to live their lives.
