CHOICES CARE have been providing services to adults who have a Learning Disability in several locations throughout the UK since its commencement in April 1995.
We currently have over 800 staff providing supported living, residential, domiciliary care and day care to people who have a Learning Disability.
We work with people who have a wide range of support needs. Some of the people we support present with complex needs associated with Autistic Spectrum Disorder, mental/physical health problems as well as challenging behaviour.
We pride ourselves in our ability to meet the unique needs of the men and women in our services and work in partnership with them, their families, and other relevant agencies through adopting a person centred approach to identify their needs and provide care.
The person being supported is central to the way in which we deliver care. Various stakeholders including families, Care Managers and health professionals contribute so we have a holistic overview of the best way to support people who use our service. Support plans are implemented, reviewed and updated regularly. The person being cared for is involved in this process in the most appropriate way for them.
